To start off, open up a new document, around the size of 300 x 300 px. Fill the background layer with White (FFFFFF). Now grab your Pen Tool with these settings. Make a shape that looks like this. Product.

Now it's time to give this shape some layer styles. Give it an Inner Shadow, and Inner Glow, and a Gradient Overlay with these settings. Product.

Now grab your Rectangle Tool with the default settings and make a shape that looks like this.

Now give this shape's layer some layer styles: an Inner Shadow, a Bevel and Emboss, and a Gradient Overlay with these settings. Product.

Now grab your Pen Tool again and make a shape that looks like this. Image. Apply the same styles to this shape as on the previous shape by right clicking on the previous shape's layer in the Layers Palette and choosing Copy Layer Style. Then right click on this shape's layer and choose Paste Layer Style. Product.

Now right click on that shape's layer in the Layers Palette and go to Duplicate Layer. Drag it up a few pixels. Duplicate this layer two more times and drag the two additional duplicates up further. Refer to this image: Product.

Now grab your Elliptical Marquee Tool with these settings. Make a selection here. Fill it in with White (FFFFFF).

Give this circle fill layer some layer styles: an Inner Shadow, a Bevel and Emboss, and a Stroke. Product.

Grab the Pen Tool once more and make a shape that looks like this. Give it an Inner Shadow layer style. Product.
